To qualify for the rebate you must purchase your machine from one of the valid retailers listed on the rebate form, and include the following:
1. The completed mail-in rebate form or a copy.
2. A copy of your sales receipt showing the select Bosch item purchased within the promotion dates listed above.
3. The original 12-digit UPC barcode from the item.
4. Requests must be postmarked by the postmark date listed above.
5. Make copies of all your submission materials and mail all of the above requirements to the address listed to the right.
